Patient PrivacyPrivacy Notice Central Vermont Medical Center follows every state and federal rule to protect your privacy and limit any unwanted access to your health information. This information belongs to you. It cannot be released to anyone, including your relatives, unless you agree.
When you are admitted as a patient to the Medical Center, you will be asked to sign a three-page form. This Consent, Release & Acknowledgment form outlines the conditions under which we can provide care for you.
In order to be a patient, you must sign this three-page form.
The Consent page cannot be altered in any way. If you request changing the words or if you refuse to sign the form, Central Vermont Medical Center cannot proceed with your treatment or testing.
The Release page offers you, the patient, the option of not being listed in the hospital's directory (this is the list of inpatients located at the volunteer desk) and not allowing your condition to be discussed with family or close friends involved with your care unless you agree.
The Acknowledgment page simply outlines the various policies that exist at CVMC and acknowledgement of the federally required "Important Notice from Medicare."
