VT Blueprint for HealthIn 2006, Central Vermont Medical Center and Central Vermont Physician Hospital Organization received a grant from the Vermont Department of Health. The purpose of the grant is to improve care delivery to community members with chronic illnesses.
CVMC Blueprint Integrated Pilot In 2009, CVMC was chosen as a Blueprint Integrated Pilot site. Our medical home practices that are participating in this pilot program are as follows:

- The Health Center, Plainfield
- Associates in Family Health
- Central Vermont Primary Care
- Waterbury Medical Associates
What This Means for Central Vermont We know from national statistics that individuals with chronic conditions do not receive all of the care they should in order to stay healthy. We also know that historically, office practices were built on an acute care model. The goal of the Vermont Blueprint for Health grant is to improve care delivery, moving from an acute care to a planned, proactive care model. We are proud to receive this grant funding which will allow us to create a model of care that will encompass new community partnerships while building new systems that better serves the needs of our community.
